Care Manager RN Per Diem Day
About The Position
Utilizing a collaborative process, the Case Manager will establish a continuum of care plan for a caseload of patients. The Case Manager will establish quality monitoring indicators, collection of data, analysis, documentation and the reporting outcomes, and liaison with patients. The Case Manager will act as a liaison to facilitate a smooth transition for the patient from acute to the next level of care. Providence caregivers are not simply valued – they’re invaluable. Requirements
- California Registered Nurse License
- 2 years Acute care clinical or comparative experience.
- CCM
- 3 years experience in community health nursing or case management.
Nice To Haves
- Bachelor's Degree in Nursing.
