About The Position

The Care Management Supervisor supervises and provides oversight to a team of personnel supporting behavioral health members and/or members with I/DD that receive care management services from Carolina Outreach. As a Supervisor of Care Management, duties will include supporting and monitoring Care Management team performance, guiding and driving positive member outcomes, and ensuring the delivery of integrated whole-person care management. The Supervising Care Manager will: Supervise administratively / clinically a team of 8 care managers; Be responsible for managing care managers’ performance and continually coach care managers on how to achieve their individual potential; Ensure that all assessments, and care plans completed by the care manager team meet program standards; Monitor and track compliance with HEDIS measures and 1915i services Deliver guidance and feedback on strategies for engaging members and working with stakeholders to support member's care plan goals; Monitor the level of member engagement with the care management team to ensure contract requirements are met.

Requirements

  • A master’s-level degree in a field related to health, psychology, sociology, social work, or another relevant human service; or
  • Licensure as an RN.
  • A master’s-level, fully Licensed care manager, or licensure as an RN and must have three years of experience providing care management, case management, or care coordination to the population being served.
