About The Position

The Care Facilitator serves as a primary point of contact for patients at ChenMed, providing exceptional customer service and administrative support. This role is crucial in ensuring a positive patient experience by managing check-ins, scheduling appointments, and maintaining patient records. The Care Facilitator acts as a patient advocate, fostering trust and communication between patients and healthcare providers.

Requirements

  • High school diploma or equivalent education required.
  • Graduation from a nationally accredited Medical Assistant program preferred.
  • Minimum of 1 year of work experience in a medical clinic or similar environment required.
  • BLS for Healthcare Providers preferred.
  • Exceptional customer service skills and a passion for serving others.
  • Working knowledge of medical insurance and electronic medical record (EMR) systems.
  • Proficient in Microsoft Office Suite, including Excel, Word, and Outlook.
  • Ability to communicate professionally with patients and staff.
  • Detail-oriented with strong listening skills and outstanding verbal and written communication skills.
  • Ability and willingness to travel locally and regionally up to 10% of the time.
  • Fluency in spoken and written English.

Nice To Haves

  • Experience in a healthcare setting or medical office administration.
  • Knowledge of ChenMed medical products, services, standards, policies, and procedures.

Responsibilities

  • Interact with patients to check them in and out of the clinic, schedule appointments, and record information.
  • Conduct lobby checks to maintain organization and cleanliness.
  • Manage schedules for primary care providers and specialists, ensuring timely appointments for patients.
  • Utilize computer systems to add patient information, print forms, and generate new patient packets.
  • Ensure accurate documentation of patient and emergency contact information.
  • Make 'love' calls and send birthday cards to patients to commemorate special events.
  • Answer phones, take messages, and direct them to the appropriate personnel.
  • Communicate information about clinical personnel to patients and others.
  • Verify patient insurance and collect copays as necessary.
  • Assist with scheduling follow-up and specialist referral appointments.
  • Escort patients to exam rooms and ensure their comfort.
  • Act as HEDIS champion by monitoring patient needs and reporting on hospitalizations.
  • Present customer service surveys to patients during checkout and escalate issues as needed.
  • Participate in daily and weekly huddles to discuss patient details.
  • Regulate transportation aspects for patients, including scheduling rides and managing deliveries.
