Since 1971, the Hampton-Newport News Community Services Board has provided the highest quality of behavioral health services to residents of the cities of Hampton and Newport News, Virginia, offering a comprehensive array of programs for both children and adults living with mental illness, developmental disabilities, and/or substance use disorders. As a Care Coordinator at the Hampton-Newport News Community Services Board, you will play a pivotal role in the Regional Crisis Stabilization Center (RCSC), ensuring seamless service delivery to consumers. You will be an integral part of a multidisciplinary team, acting as the liaison between the RCSC and community programs to facilitate effective discharge planning and aftercare coordination. Your responsibilities will include coordinating referrals, advocating for clients, and completing essential case management tasks such as benefits application and documentation in the electronic record. Additionally, you will collaborate with various internal and external providers, review pre-screening evaluations for care appropriateness, and make informed treatment recommendations. By developing client service plans and providing direct crisis intervention support, you will help enhance the overall well-being of individuals in crisis, all while maintaining a focus on advocacy and client confidentiality in accordance with established policies.

Requirements

  • Master’s Degree in Human Services
  • One (1) year experience providing service collaboration and coordination, psychoeducation and behavioral health skills trainings to the client population assigned
  • Must be registered with the Board of Counseling to practice as a QMHP-A
  • Valid VA Driver's License
  • Strong organizational skills are necessary for maintaining accurate documentation and coordinating with various stakeholders
  • Effective communication skills will be critical when interacting with clients, families, and community providers
