The Adult Criminal Justice and Treatment Services (ACJS) division at TASC supports clients with substance use disorders, helping them rebuild their lives to become healthy and self-sufficient. ACJS provides screening and assessments, treatment program placement, progress monitoring, and support services like insurance enrollment, state ID acquisition, and employment resources. The Care Coordinator role is part of the Collaborative Bridges team, an innovative care coordination pilot program. This program connects individuals and families with substance abuse and mental health needs upon hospital discharge, providing supportive care coordination until they are linked to long-term community providers or their needs are resolved. The position involves educating and guiding patients on the critical role of primary care providers in maintaining overall health, emphasizing preventive care, chronic condition management, and coordinated specialized treatments. Care Coordinators encourage patients to establish relationships with community-based primary care providers for continuity of care, timely medical interventions, and trusted health guidance. They also provide resources and support to help patients navigate the healthcare system, select appropriate providers, and understand the value of regular check-ups for long-term wellness.
