Flexible scheduling for many roles Position Summary Ability to engage children/youth/adults in care. Provide case coordination and care management for assigned clients. Document services accurately in accordance with Medicaid and agency standards. Participate in care planning and multidisciplinary meetings. Collaborate with identified sources and service providers to complete timely referrals. Maintain current knowledge of community resources that address the needs of individuals served. Monitor client progress and reassess level of care needs. Identify and resolve barriers to treatment engagement. Maintain regular communications with stakeholders to coordinate services. Adovocate for client access to appropriate services and supports. Serve as a connector to services based upon identified diagnostic assessments. Support continunity of care across systems (behavioral health, school, child welfare, medical). Perform other duties as assigned.
