About The Position

Comprehensive Healthcare provides innovative behavioral health and integrated healthcare services to clients of all ages. In this role, the Health Home Care Coordinator works with individuals who have chronic health conditions and who are transitioning from inpatient stay or are high users of the healthcare system. Using excellent communication skills, the Care Coordinator will provide services and coordination to clients to ensure continuity of care across health and social service programs. This position requires strong interpersonal and organizational skills in order to build rapport with clients and to coordinate with various healthcare providers and community services. Comfort and confidence in addressing medical topics and in doing home visits is essential. This position is part of our Health Homes team in Pasco and will provide care coordination services throughout the Tri-Cities area. Schedule: Monday-Friday, 8am to 5pm

Requirements

  • Bachelor's degree in social work, psychology, or related field
  • Must be eligible for registration as Agency Affiliated Counselor
  • Must have valid driver's license and vehicle liability insurance

  • It is the policy of Comprehensive Healthcare to ensure that employees are provided the equipment necessary to protect their health from respiratory disease in accordance with applicable laws and regulations.
  • Respirators, training, and medical evaluations are provided at no cost to the employee during normal working hours.
  • The medical evaluation will determine the employee’s ability to use a respirator and is required prior to completing fit testing of the respirator.
  • Fit testing will be performed using the same make, model, style, and size of respirator that will be used by the employee when at work.
  • A respirator is designed to achieve a very close facial fit and very efficient filtration of airborne particles.
  • The edges of the respirator are designed to form a seal around the nose and mouth.
  • To achieve this, employees must (a) arrive for fit testing clean shaven around the sealing surface of the mask; (s) have nothing to eat or drink other than plain water for 20 minutes prior to the appointment; and (3) have not smoked, vaped, or chewed gum for 20 minutes prior to the appointment.

Responsibilities

  • reaching out to and meeting with potential clients to recruit them into the program
  • completing care management assessments and monitoring client progress towards improving self-management of their chronic illness
  • collaborating with the client to develop an individualized health action plan that includes all systems of care with which the client is involved
  • coordinating with primary care providers & other treatment teams to facilitate communication with the client
  • coordinating with medical facilities at all levels to ensure the continuity of care as clients transition between care systems
  • providing education & coaching for the client and their support system regarding medications, health behavior changes, and accessing community resources & services
  • working independently to meet clients in their homes or out in the community
  • entering clinical data into multiple systems accurately & efficiently
  • completing all documentation in a timely, client-centered manner
