Requirements

A minimum of an Associate's Degree (Bachelors preferred) in a Human Service field or full certification as an Addictions Counselor
Associate's Degree: Five (5) years
Bachelor's Degree: Three (3) years
Master's Degree: Two (2) years
Certified Addictions Counselor (CAC): Five (5) years

Responsibilities

Responsible for conducting intake interview and orientation to care coordination, with client prior to discharge from residential level of care
Responsible for liaison with clinical staff for review and verification of recommended services, referral to clinically recommended level of care, and collateral contact information
Completion of Recovery Plan in coordination with client and primary clinical counselor
Responsible for identifying health care and recovery support needs with client and initiating referrals
Responsible for assisting clients in getting to the next level of care (if applicable) by removing barriers, providing reminders, and conducting outreach when appropriate
Responsible for developing and maintaining relationships with services and programs. These linkages will include but are not limited to treatment, health care, employment, legal, housing, child welfare, social services, peer-support providers, etc.
Establish and maintain care coordination services and contact schedule
Maintains client records and completes all other required documentation
