Phoenix Home Care and Hospice is seeking a Care Coordinator for our Private Duty Nursing team in Columbia. This is a fast paced, people focused role where organization, urgency, and communication matter every single day. Responsibilities

  • Create and maintain nursing schedules
  • Schedule nurses based on care plans and staff availability
  • Review client agreements and care plans to ensure appropriate service coverage
  • Respond to urgent staffing needs
  • Enter and maintain accurate staff and client information in the database
  • Troubleshoot issues and provide timely resolution
  • Handle a multi line phone system with professionalism
