The Care Coordinator will: Conduct timely and accurate assessment of the patient/ client clinical, social, functional and continuing care needs through direct observation, data analysis and interviews in order to evaluate the need for intervention. Establish immediate, short term, and long-term care needs with the patient. Engage in quality through value-based care delivery. Engage in and promote communication at all levels. Demonstrate competency and understanding of the specific knowledge and skills required for effective care coordination.
