About The Position

Mental Health Services Care Coordinator A MENTAL HEALTH CARE COORDINATOR is a key member of a client’s multidisciplinary team. You provide direct support, education, referral and outreach services to a caseload of clients diagnosed with mental illness. Care Coordinators, along with other team members, often meet with clients in their homes and out in the community to provide services that aid them in living fulfilling lives not dominated by their mental illness. A Care Coordinator will actively participate in team meetings to ensure the best plans and services for each client as well as engage other community resources and services to assist in addressing an individual’s unique needs, as appropriate. Care Coordinators conduct assessments and provide direct support, education, referrals, advocacy and outreach services to clients. They collaborate in team meetings to ensure the best individualized treatment plans and comprehensive services for each client, including medicine management, therapy, skills training, employment assistance, housing support, family education and connecting them with other community resources as appropriate. NO NIGHTS OR WEEKENDS!!

Requirements

  • A bachelor’s degree in a Human Services or related field is required
  • Can work effectively in a team environment but can also function independently and be a self-starter
  • A responsible, caring and organized individual with strong organizational, communication and interpersonal skills
  • Driving and criminal background checks required!

Nice To Haves

  • At least one year of previous experience with mental illness (preferred)
  • Knowledge of area resources is a plus

Responsibilities

  • Monitor patient progress and document observations accurately
  • Assign responsibilities, provide services, motivation and constructive feedback, while ensuring accountability
  • Maintain supportive relationships with individuals and their families while encouraging independence
  • Keep individuals and their families informed of their rights and act as an advocate when appropriate
  • Adhere to HIPAA regulations and maintain confidentiality of sensitive information
  • Provide emotional support through active listening and discussing concerns
  • Encourage clients to make good choices and achieve personal goals
  • Collaborate with the care team, including outside partnerships, to ensure comprehensive patient care and implement effective interventions
  • Consistently act with integrity, while demonstrating a strong commitment to quality services and the best interests of clients served.
