Requirements

  • 2 years as Care Coordinator or related role utilizing the IOCP model - Required
  • 3 years as a Care Coordinator working with the IOCP model in an outpatient clinical setting. - Required
  • Basic Life Support (BLS) Basic Life Support (BLS) Certification - Required
  • Fingerprint Clearance Card - State Current and valid State of Arizona Finger Print Clearance Card Current - Required
  • High School Diploma or GED - Required

Nice To Haves

  • Associate's Degree or health care related advanced education. - Preferred

Responsibilities

  • Assisting and mentoring Care Coordinators in the new practice on-boarding process, identifying readmission risk and barriers to care outpatient including home support, medication management, expectation, etc.
  • Provides coverage, educates and gives direction to Care Coordinators.
  • Completes, organizes and schedules educational and certification classes for continued learning for team members.
  • Mentors and precepts new Care Coordinators during on boarding process Ensures effective post-hospitalization telephonic monitoring, Supervisits and/or Office Visits.
  • Maintains the community resource information.
  • Communicates information regarding inpatient stay and discharge plans to patient’s PCP/office care manager/health coach.
  • Assures effective transition and final hand-off to the patient’s PCP and his/her office based care manager/health coach.
  • Coordinates with organization(s) (employee plan) or Payer Care Management regarding support desired/required.
  • Promotes a collaborative process and communication (either verbal or written) between all health care team members, including patients/clients, families and significant others to ensure the process of integrated care services are targeted, appropriate, and beneficial to the population served in the Comprehensive Care Coordination Program.
  • Maintains a working knowledge of applicable laws and regulations, government (Medicare, Medicaid, SSI, and SSDI) and insurance benefits.
  • Maintains and manages their caseload while assisting in the orientation of new Care Coordinators.
  • Performs other duties as assigned.
