About The Position

Responsible for regular Communications with the members (minimum of one visit per year, quarterly phone calls and as needed. In conjunction with the program manager, responsible for networking activities within the market area designed to enhance community awareness of the Carlyle at Home program. In conjunction with the program manager, develops a network of sub-contracted service providers. Interviews, selects and negotiates contracts with all service providers, including home care agencies, transportation providers, etc. necessary to ensure exceptional delivery of services. Obtains and reviews required documents from prospective service providers to ensure they meet Carlyle at Home standards. Develops a network of vendor partners for referrals for non-covered services. Maintains the network and manages the referral process. Schedules and conducts pre-admissions assessments of prospective members. Reviews and organizes medical records to present to membership committee. Serves on the Membership Committee. Conducts new member meetings. Ensures that new members are welcomed into the program quickly and efficiently. Conducts annual and as needed assessments of members to determine the need for services. Visits members in the hospital, in rehab centers or at home to determine the need for services and to provide reassurance. Manages the successful placement of appropriate services to address members’ needs. Communicates with members regarding what Carlyle at Home will pay for (what is “covered”), and members’ financial responsibilities Follows up with members to determine their satisfaction with services. In conjunction with the program director, manages any quality monitoring initiatives. Serves on the Quality Monitoring Committee. Participate in regular information seminars, including conducting presentations to small groups of prospective Members.

Requirements

  • Bachelor’s Degree in Nursing, Gerontology, Social work or Social work-related fields i.e. Human Services or Psychology
  • Professional Licensure verification (when applicable)
  • Valid Driver's License
  • Motor Vehicle Driving History Clearance
  • Educational skills and experience in working with aging
  • Knowledge of sales and marketing
  • Excellent verbal and written communication skills
  • Must be familiar with Windows operating systems
  • Must be able to use visual near and distance acuity
  • Must have verbal ability to communicate in person and over the phone or radio
  • Must be able to assist co-workers in transfers and transport supplies, patients/residents, or equipment using good body mechanics
  • Must have functional range of motion of the cervical, thoracic and lumbar spines, upper and lower extremities with a grip strength of 60-80 pounds, specific to the job class evaluation
  • Must be able to forward reach, overhead reach, bend, squat, kneel and change from one position to the other rapidly
  • Must be able to lift 25-60 lbs. and carry 25-50 pounds specific to job class evaluation
  • Must be able to tolerate high noise, busy atmospheres and noxious odors
  • Must be able to work in noisy environments and be self-directed to complete jobs as ordered
  • Must be able to remain calm under stressful situations
  • Must be able to assess and respond to multiple situations simultaneously
  • Must be able to assist co-workers in multiple overlapping tasks simultaneously
  • Must be able to prioritize job tasks and organize work space to be efficient and conducive to completing work tasks as assigned in a timely manner
  • If job class is identified as falling under POPE C, the individual must pass the cardiac recovery test
  • Works in hospital and clinical environments
  • May work in extreme heat with hot materials
  • Must be able to utilize appropriate universal and specific precautions for protection from potential exposure to infectious or communicable diseases

Nice To Haves

  • Bachelor’s Degree Disciplines with degrees in Nursing, Physical Therapy, Occupational Therapy, or Speech therapy with related experience
  • 2-3 Experience related to field of study
  • Experience conducting a variety of assessments, e.g. MMSE, 7-minute screen, FIM measures, Geriatric Depression Screen, etc. is preferred
