Working closely and collaboratively with the HUH inpatient and Consultation-Liaison and Addiction Medicine Consult services and the ambulatory Mental Health Clinic (Behavioral Health Clinic) teams, the Care Coordinator assesses clients’ needs and support systems; facilitates timely, coordinated access to and continuation of medically appropriate substance use disorder, behavioral health, and general health care health and support services; assists in expediting appropriate discharge from inpatient facilities, as medically appropriate, and assists in preventing unnecessary hospitalizations; and establishes and maintains relations with case managers, healthcare providers, social services and other agency representatives to bridge the gaps between health, social service, entitlements and other systems.
