About The Position

This position is Hybrid role with work onsite and offsite and will require a flexible schedule which includes working evenings and/or weekends, if necessary. The Care Coordinator is responsible for providing care management to a group of patients and assists in prevention efforts to help patients achieve and maintain their optimal level of health. Using a team based approach, the care coordinator will act as a resource to patients and health care staff to ensure patients receive all the care and services they require. Care Coordinators must be proactive, well-organized, present a friendly demeanor and function well in a fast passed clinic environment while providing best patient care services. This position is hybrid, with most work completed on site and from home. Care Coordinator must be available to work on site when required.

Requirements

  • Associates Degree or equivalent work experience, required.
  • Three or more years of clinical experience, required.
  • Bilingual English/Spanish, required

Nice To Haves

  • Experience as a Medical Assistant, Case Manager or Health Educator, preferred.
  • Previous experience with Electronic Health Records, preferred.
  • Knowledge of Microsoft Office (Excel, Word), preferred.

Responsibilities

  • Communicating with care team, reviewing patient charts, identifying gaps in care, identify areas of need, scheduling appointments, participating in the establishment of the care plan, ensuring fidelity to the care plan.
  • Facilitate communication among patients, providers, nurses, medical assistants and outside resources, related to the care of patients.
  • Maintain timely and consistent communication with patients regarding their health care and resource needs.
  • Use assessment results and provider input to refer patients to community resources (e.g. health insurance services, public assistance, transportation, child care, employment, nutritional/exercise classes, etc.) and to ensure services are received.
  • Help patients to connect to primary care, specialty, and ancillary (e.g. lab/diagnostic) services by providing information and making appointments as needed.
  • Encourage patients to become actively engaged in their own health through calm, compassionate communication, and by utilizing motivational interviewing techniques.
  • Engage in community outreach with neighborhood schools, churches and other business partners.
