We are seeking a Care Coordinator to join our team. In this role, you will work collaboratively with patients to determine their medical needs, develop the best course of action, and oversee their treatment plans, ensuring each client gets high-quality, individualized care. The ideal candidate is compassionate, patient, and knowledgeable about healthcare practices. The Sobering Center, founded in 2018, provides the community with a safe place to recover from intoxication in lieu of the jail or the emergency room, and refers individuals to community-based resources. To date, it has served over 7,000 individuals and continues to evolve its services to meet community needs. Clients reflect the Austin/Travis County community, including college students, festival goers, partakers of downtown entertainment, and the chronically homeless. All are welcome, and staff individualize interventions. Substance use disorder (SUD) and alcohol use disorder are leading causes of preventable illness and early death, with only 10% of affected Americans receiving treatment. The Sobering Center provides counseling to those who identify with or meet the criteria for SUD, diverting publicly intoxicated individuals from the criminal justice or medical systems and providing direct intervention to help them get the assistance they need.
