Our vision is simple and bold: To show the world what care should feel like. The Heart of This Role The Care Coordinator is the bridge between a provider’s visit and a patient’s ongoing health. This role ensures patients stay connected to care, understand what comes next, and receive the preventive and follow-up services they need to stay healthy. Care Coordinators actively manage a provider’s patient panel, conduct outreach, close care gaps, and help patients navigate the healthcare system with clarity and confidence. This is not just a scheduling or outreach role. It is a relationship-based position that directly impacts quality outcomes, continuity of care, and whether patients remain engaged in their health. What This Role Looks Like, Day to Day In this role, CONNECTION means: You build trust through consistent, proactive outreach. You communicate clearly with patients about preventive care, follow-ups, and next steps. You help patients understand why appointments, screenings, and wellness visits matter. You meet patients where they are, even when barriers exist. OWNERSHIP means: You actively manage an assigned provider’s patient panel. You identify and close gaps in care using reports and dashboards. You schedule and track preventive visits, chronic care follow-ups, and wellness visits. You document outreach accurately and consistently in the EHR. You escalate high-risk or complex patient needs to the RN Care Manager when appropriate.
