CARE COORDINATOR
About The Position
The MBH Care Coordinator is responsible for the overall internal growth of the outpatient programs. The Care Coordinator identifies, educates, and assists in facilitating individuals from the inpatient unit to the outpatient programs. The Care Coordinator also works to improve the overall patient flow and access to outpatient care. The MBH Care Coordinator is also responsible for discharge follow up phone calls to all patients discharged from the inpatient hospital units. The Care Coordinator will contact the patient and/or legal guardian to check in with the patient regarding any questions related to discharge instructions, review follow-up appointments and provide contact information for any future questions or concerns.
Requirements
- A bachelor’s degree in a behavioral health area, social work, or marketing or related field is preferred.
- Minimum of 1 to 2 years’ experience in a mental health setting required
- experience in an inpatient psychiatric facility/unit is preferred.
