About The Position

Responsible for providing care coordination services for our unhoused and unsheltered populations. This role supports clients in meeting recovery goals through education, outreach, and connection to Medication Assisted Treatment (MAT) and additional community-based supports.

Requirements

  • High school diploma or completion of G.E.D required
  • At least one year of experience in care coordination or case management within the mental health field, substance use disorder field, or program area required
  • Valid Colorado driver’s license or the ability to obtain one within 30 days of hire required; must maintain a clean driving record. This role requires occasional travel to client homes and community partner sites.
  • CAT, CAS, CAC III, LAC or preferred willingness to pursue

Nice To Haves

  • Bachelor’s degree in psychology, social work or related field from accredited College or University preferred
  • Prior experience working directly with individuals experiencing homelessness, including hands-on case management coordinating services, securing housing, and connecting individuals to behavioral health and community resources is highly desired
  • Demonstrated experience working in a community-based behavioral health setting, providing direct support or services to individuals with mental health and/or substance use challenges preferred
  • Bilingual/bi-cultural preferred

Responsibilities

  • Provide care coordination services in collaboration with clients and care teams.
  • Engage with community partners including emergency departments, primary care, acute care systems, crisis response teams, and behavioral health teams to support continuity of care.
  • Conduct outreach through referrals or cold outreach to engage clients in services.
  • Participate in Certified Addiction Specialist coursework.
  • Work collaboratively with SHP staff, CO-SLAW, community health centers, and partner agencies to support smooth transitions from acute care settings.
  • Coordinate with internal teams and community agencies to identify resources, address service gaps, and support continuity of care.
