They serve as the operational and relational bridge between clients, caregivers, referral partners, and leadership-ensuring care plans are executed with precision and heart. The Care Coordinator is responsible for ensuring the seamless delivery of care by developing, monitoring, and evaluating individualized home care plans. This role ensures both client satisfaction and caregiver success through regular communication, field engagement, and operational follow‑through. You'll partner closely with clients, caregivers, and internal team members from intake through ongoing service. Requirements

  • Minimum 2 years of home care experience - REQUIRED
  • Strong communication and customer service skills - REQUIRED
  • Valid driver's license with reliable transportation and active insurance
  • Ability to work at least 40 hours per week
  • Ability to perform essential job functions with or without reasonable accommodation

Responsibilities

  • Contact all referral leads the same business day they are received (or the next business morning for after‑hours referrals)
  • Track referrals and conduct consistent follow‑up
  • Educate new referrals on the intake and enrollment process
  • Manage assigned private pay and state waiver clients
  • Conduct initial client intake visits and develop customized plans of care
  • Complete service agreements, care plans, and new client documentation
  • Meet caregivers in the client's home to review the care plan and expectations
  • Ensure each client has and uses a daily home care logbook
  • Conduct home visits and 90‑day client evaluations
  • Monitor caregiver performance and adherence to policies and care plans
  • Address caregiver supply needs when identified
  • Collect required documentation from clients and caregivers during visits
  • Document and escalate client complaints, implementing resolution plans with leadership
  • Coordinate and maintain service authorizations with managed care organizations
  • Assist with scheduling challenges to ensure open shifts are staffed
  • Participate in on‑call rotation (minimum one week per month)
  • Maintain reliable, punctual attendance both in‑office and in the field
  • Perform additional duties as assigned to support quality care delivery
