About The Position
Saint Joseph's Medical Center is seeking a full time Care Coordinator in our Care Coordination Department. Will assess client needs and integrate care coordination services into total care of clients. Perform Health Home and DCMH SPOA assessment. Incorporate collaborative and client-centered health home services into clients' care plans.
Requirements
- Bachelor's Degree in Human Services or Related Field
- Must have valid Driver's License and own vehicle
