Academic Req: Bachelor’s degree Work experience: Required – 2 years’ experience in child welfare or crisis response Summary: The coordinator is responsible for assessing whether a minor client, identified or recovered, meets protocol eligibility to launch services. Coordinators are responsible for setting up and facilitating all care coordination meetings. Coordinated responses include advocates, law enforcement, medical, child welfare, placement, mental health providers and if available child advocacy centers.

Requirements

  • Pass a pre-employment drug screen and random drug screens and maintain emotional control and professional composure always.
  • Acquire and maintain licenses and certificates required for the position which may include but is not limited to: ☐ Trust Based Relationship Intervention (TBRI), ☐ CPR ☐ First Aid, ☐ CSE-IT Certification, ☐ State Driver License ☐other certifications as applicable
  • Possess current auto insurance and provide verification upon request.
  • Provide proof of work eligibility status upon request.
  • Pass pre-employment and biennial criminal background checks.
  • Demonstrate the ability to: Respond sensitively and competently to the service population’s cultural and socio-economic characteristics
  • Advanced training in crisis management
  • Communicate effectively in writing and verbally in English.
  • Work in a fast-paced environment and maintain emotional control and professional composure always
  • Maintain computer literacy required to meet the responsibilities of the position

  • Demonstrate a working knowledge of all BCFS HHS policies and procedures.

Responsibilities

  • Participate in hotline rotation duties and document all received information
  • Documenting and maintaining all data for grant metrics into appropriate database within 48 business hours of occurrence including report/notification information, demographics, services provision, and investigative steps
  • Document any crisis response within 24 hours
  • Assess whether the identification or recovery of the minor meets agreed upon protocols to launch coordinated services
  • Complete CSE-IT assessments as needed
  • Secure and retain the release of information and consent for care coordination services
  • Dispatch advocate response, as determined by working CCT protocol
  • Set up Rapid Response Team meeting with partners within 24 hours of notification of the identified or recovered minor
  • Establish and maintain contact with law enforcement agencies and key stakeholders to initiate meetings and facilitate ongoing monthly coordination of services
  • Attend MDT meeting, as needed, to represent clients and Care Coordination Team
  • Provide facilitation and support for Advisory Council meetings, including preparing agendas, communicating with attendees, and documenting meeting minutes, as required or directed by the supervisor.
  • Manage all requirements related to Care Coordination meetings, such as but not limited to, developing agendas, coordinating in person and/or virtual meetings and logistics, and ensuring timely notification to all attendees
  • Implement BCFS HHS safety protocols in case of an emergency
  • Participate in weekly internal staffing’s, supervisions, Care Coordination meetings, and community meetings
  • Create information tracking system, tracks requested volunteer information, and supports quarterly report development
  • Establish subject matter expertise on community resources and support program leadership in conducting community trainings as needed
  • Maintain confidentiality in all areas of the service population and program operation
  • Maintain BCFS HHS professional and ethical standards of conduct outlined in employee handbook including demonstrating respect for agency staff, the service population, and community members and always complying with required dress code
  • Travel across regions with occasional overnight stays, if needed
