About The Position

The Care Coordinator is responsible for connecting clients to resources and services provided by Meals on Wheels San Antonio and the wider community to support their independence, safety, and wellness. The Care Coordinator also responds to community members’ requests for assistance and referrals to community resources. This position will support partnering agencies to improve available resources and services for clients. Implement established workflows and processes to respond with person-centered approach to assist clients. Maintains partnerships with community agencies to improve referrals for services. The Care Coordinator tracks and maintains data required for documentation and funding purposes.

Requirements

  • Must have reliable, personal vehicle and a valid Driver’s License, good driving record, and applicable insurance coverage required by the company policy.

  • Have mobility to work in a standard office setting and to use standard office equipment, including a computer; vision to read printed materials and a computer screen; and hearing and speech to communicate in person and over the telephone.
  • Able to work at a desk and computer screen for extended periods of time; anticipated that the person may spend several hours of each day seated at a computer.
  • Able to attend meetings at various locations within and away from the city.
  • Able to move freely in and out of vehicle and climb stairs as needed to complete client visits as necessary.
  • Works in highly stressful environment dealing with a wide variety of challenges, deadlines and diverse employee population.
  • Able to occasionally lift up-to twenty-five (25) pounds to move office supplies or other equipment.
  • Two years’ experience in elderly care, social services setting, or making client home visits.
  • Strong analytical and data management skills as evidenced by capacity to prioritize multiple tasks and role components.
  • Excellent office administration skills with an advanced knowledge of Microsoft Office programs, particularly Excel.
  • Maintain client, provider, and referral agency confidentiality; comply with all HIPAA regulations.
  • Working under pressure – ability to complete assigned tasks in stressful situations.
  • Ability to work independently and exercise appropriate judgement in interactions with clients, medical providers, Community Based Organizations (CBO) providers, contractors, and clients’ caregivers.

Nice To Haves

  • Bachelor’s degree preferred.
  • Three years’ managerial experience preferred.
  • Bilingual (Spanish/English) preferred.
  • Experience working with seniors and/or people with disabilities preferred.

Responsibilities

  • Deliver care coordination to clients that is timely, effective, equitable, and person-centered.
  • Develop effective working relationships with drivers, meal recipients and their caregivers, and meal recipients’ medical and non-medical providers.
  • Implement and maintain data collection of drivers’ observations, as reported in the Conditions of Concern tool, and actions taken in response to reports.
  • Utilize a decision tree to tailor response to observations reported in the health and safety screening assessment.
  • Manage all information and data collected electronically with the scope of the position with complete confidentiality and security.
  • Maintain confidentiality of meal recipients’ health and safety information.
  • Compile and report information and date for summary reports.
  • Research, update, and share comprehensive referral resource database.
  • Establish partnerships with local non-profit, medical, and other organizations who provide services to clients.
  • Other duties as assigned by management.
