Sandra Eskenazi Mental Health Center, Indiana's first community mental health center, provides comprehensive care for emotional and behavioral problems, including severe mental illness and substance abuse. The Sandra Eskenazi Mental Health Center offers both inpatient and outpatient services, including several outreach centers as well as clinic- and community-based services. About the Adult Outpatient Program (AOP)Adult Outpatient (AOP) serves adults experiencing schizophrenia and schizoaffective disorder along with other mental health and substance use disorders. Adult Outpatient has clinical psychologists that lead the AOP teams, in collaboration with a clinical supervisor.Where You Fit In:The Care Coordinator provides skills training and development to clients as required by medical necessity. In collaboration with the treatment team, the Care Coordinator conducts necessary client assessments as well as formulates, develops and revises treatment plans. The Care Coordinator advocates for clients to ensure access to needed services which include developing community relationships and resources. What to ExpectThis is a community-based position. While some clients may choose to be seen in the clinic, the role involves spending a significant amount of time providing home-based services and support in the community. The Care Coordinator creates their own community schedule, providing flexibility to focus each day by geographical areas. All clients are within Marion County, and we reimburse for mileage.
