Care Coordinator – Support Services
 Location: Van Nuys, CA.| Full-Time

thrive? 360 Behavioral Health is looking for a Program Coordinator who thrives
in a fast-paced, rewarding environment. You’ll oversee Direct Support
Professionals, collaborate with families and Regional Centers, and ensure every
client receives high-quality, compassionate care.

Requirements

• 1+ year clinical experience working with individuals with diverse needs
• Strong leadership, organization, and communication skills
• Ability to work independently and collaborate with a multidisciplinary team
• Comfortable in diverse home environments and culturally responsive
• Bilingual Spanish Required
• High School Diploma/GED required; pursuing bachelor’s degree preferred
• Must pass background check/Live Scan
• TB clearance, CPR/First Aid, and Crisis Prevention Certification

Nice To Haves

• Supervisory experience (preferred)

Responsibilities

• Supervise and coach DSPs to deliver exceptional services
• Ensure families receive consistent, high-quality care
• Collaborate with Regional Centers, clients, and behavioral agencies
• Conduct staff meetings, professional development, and utilization reviews
• Provide clinical guidance and mentorship
• Maintain accurate documentation and communication

• Required Availability- Monday-Friday 8:00am-5:00pm evenings and weekends as needed.
