The Association for Individual Development (AID) is a non-profit organization whose mission is to empower people with physical, developmental, intellectual, mental health challenges; those who have suffered a trauma; and those at risk, to enjoy lives of dignity and purpose. We are looking for a Care Coordinator Tier 2 MHP/QMHP Pathways to Success who demonstrates this mission and wants to work for an organization that makes a difference in the community. Schedule: Monday through Thursday 11:30am - 8pm; Fridays 8am-4:30pm. Case Manager Mental Health Professional: $23.50 Hourly (Bachelor's degree Required) Case Manager Qualified Mental Health Professional: $25.75 Hourly (Master's Degree Required)
