Care Coordinator - RN or LPN
About The Position
Demonstrates skill in effective communication. Demonstrates skills in caring and is patient. Is flexible, detailed oriented and able to multi-task. Demonstrates the ability to work independently and know when to prioritize assignments. Demonstrates the ability to develop and maintain collaborative working relationships with other nurses, the personal care aides and the other staff at Clark Meadows. Exhibits leadership skills through the supervision, education and oversight of the Personal Care Aides. Actively guards the confidentiality of sensitive information including but not limited to the patients, staff and the health system. Demonstrates the knowledge, skills and ability to provide care appropriate to the age of the residents. Ability to live the CARES values at all times
Requirements
- Required New York State license as a Licensed Practical Nurse or Registered Nurse
- Basic First Aid Certification
- A graduate of an approved school of nursing
- One year of nursing experience in an enriched living, adult community, similar housing environment or long term care.
Responsibilities
- Exhibits leadership skills through the supervision, education and oversight of the Personal Care Aides.
- Actively guards the confidentiality of sensitive information including but not limited to the patients, staff and the health system.
