There will be a focus on family-driven care and utilizing a strengths-based approach, with an effort to emphasize and increase natural supports. Participants will be Medicaid/MCO enrolled, be under the age of 21, have a severe emotional disturbance, and meet criteria based on the state’s decision support criteria. Actively contribute as a team member to maintain a caseload for stabilizing clients in the community and linking clients to long-term services. Provide in person, video, and telephonic outreach and care coordination to participants and families. As an Intensive Care Coordinator, you are designated to only serve youth in Tier 2 and maintain an average caseload of 16 youth.
