About The Position

PRIMARY PURPOSE: The Care Coordinator Manager is responsible for the supervision of Care Coordinators, providing counseling and therapeutic clinical services to patients, providing counseling and linkage assistance. This position is also responsible for planning, organizing, developing, and directing the Care Coordinators in accordance with policies and procedures, current federal, state, and local standards, guidelines, and regulations governing our facility to assure that our facility is maintained in a safe and comfortable manner. ESSENTIAL JOB FUNCTIONS: Hires, trains, and supervises care coordinators and provides consultation, advice, and guidance. Ensures program staff complete required, appropriate, and needed training. Monitors and assures quality service provision and ensures that individual’s needs are met through review of monthly notes, life plans, case reviews, incident report reviews, review of Satisfaction Surveys and other evaluations. Reviews care coordinator’s documentation. Manages funding allocation for the program, including analysis of utilization of units of service to ensure budgetary requirements. Assigns and manages Care Coordinator’s caseloads. Commits to a respectful, just, and supportive environment for individuals and coworkers, aligning with the company’s commitment to diversity, equity, and inclusion. Reviews and processes Incident Reports/Events and ensures appropriate intervention and follow-up. Reviews/approves timesheets and mileage forms for program staff. Provides direct clinical services to people receiving services, as needed. Reviews applications and documents that require a supervisor’s approval. Reviews service documentation to ensure regulatory standards are met. Oversees the process of Person-Centered Planning. Oversees maintenance of program individual’s files and program records. Ensure compliance with CMS, Ryan White, quality-assurance standards, and procedures. Participates in any committee as requested by the supervisor. Demonstrates understanding and proficiency with EPIC and PROVIDE. Ensure that services provided are quality driven, cost effective, and culturally appropriate. Other related duties, as may be assigned by the Director of Care Coordination.

Requirements

  • Ability to communicate effectively in writing using the English language, with or without the use of auxiliary aids or services. Effective interpersonal skills to maintain rapport with clients, staff, and members of the community.
  • Ability to interact and work with diverse populations.
  • Awareness that FoundCare provides comprehensive health education, which may involve exposure to explicit content as part of the agency’s mission.
  • PC proficient.
  • Proficiency in communication technologies, EMR use (EPIC desired), and reporting.
  • Ability to endure short, intermittent, and/or long periods of sitting and/or standing in performance of job duties.
  • Accomplish job duties using various types of equipment/supplies, e.g. pens, pencils, calculators, computer keyboard, telephone.
  • Ability to lift and carry objects weighing 25 pounds or less.
  • Ability to travel to other FoundCare locations and perform job duties.
  • Ability to travel to other locations to attend meetings, workshops, and seminars, plus travel to other FoundCare departments and FoundCare conference rooms.
  • Bachelor’s degree from an accredited institution is required.
  • Minimum of two (2) to three (3) years in a leadership role.
  • Previous work experience with educating, mentoring, and developing team members.
  • Minimum of two (2) to three (3) years of clinical experience in clinical practice, hospital, or community resource setting.
  • Previous EMR experience (EPIC is preferable).

Nice To Haves

  • Medical Assistant Certification, Social Services Coordinator
  • Previous Chronic Care Coordination experience or social services experience
  • Local knowledge and experience working with connections in community healthcare and available social resources.
