At FBH we have a variety of client services, including outpatient, inpatient, community outreach, crisis services, partnerships with schools, law enforcement and more. Our evidence-based, culturally appropriate, trauma informed behavioral health services address the diverse needs of those we serve. Frontier Behavioral Health contracts with Molina Community Health Plan of Washington, Action Health Partners, and Medicare DSNP plans to provide Care Coordination to individuals who have multiple chronic conditions, have been identified as medically vulnerable and are at risk for ongoing high usage of emergency care. Care Coordinators are responsible for engaging with assigned members, assessing p a t i e nt n ee ds, p r oviding education, support, coordination of care and referrals to community resources, with the goal of helping members attain or improve self-management of their conditions. The Care Coordinator is responsible for creating and reinforcing Health Action Plans to address social and chronic medical and behavioral conditions. Care Coordinators are also responsible for coordinating referrals to community services and clinically indicated services such as social services, mental health care, substance abuse treatment and long term supports and services. Most work is off-site, typically in members’ homes.
