About The Position

Responsible for providing a blend of care coordination and operational support to individuals accessing substance use disorder (SUD) treatment services throughout Larimer County. This role tracks client progress toward individualized goals, ensures compliance with regulatory requirements, and supports coordinated care in collaboration with multidisciplinary and multi‑agency partners. The position also manages harm‑reduction supply systems across the agency, including ordering, tracking, and distribution processes. This role works closely with Northern Colorado Health Network, Northern Colorado Health Alliance, and other community partners to enhance client navigation, improve continuity of care, and strengthen community collaboration. The ideal candidate demonstrates cultural humility and effectively engages individuals from diverse backgrounds, identities, and communities.

Requirements

  • High school diploma or completion of G.E.D required
  • At least one year of experience in care coordination or case management within the mental health field, substance use disorder field, or program area required
  • Experience with mild to severe mental illness and substance use disorders.
  • Experience navigating systems of care.
  • A desire to work with historically marginalized communities.
  • Valid Colorado driver’s license or the ability to obtain one within 30 days of hire required; must maintain a clean driving record. This role requires occasional travel to client homes and community partner sites.
  • CAT, CAS, CAC III, LAC or willingness to pursue preferred

Nice To Haves

  • Bachelors (BA/BS) Degree in psychology, social work or related field from accredited College or University preferred
  • Demonstrated experience working in a community-based behavioral health setting, providing direct support or services to individuals with mental health and/or substance use challenges preferred
  • Bilingual/bi-cultural preferred

Responsibilities

  • Provide care coordination services on behalf of and in collaboration with clients.
  • Maintain and track supply ordering and distribution of Harm Reduction supplies.
  • Engage with various stakeholders and community partners as needed to coordinate care including but not limited to emergency departments, health services, primary care settings, acute care systems, crisis response teams, and behavioral health teams.
  • Outreach clients on a referral and/or cold outreach basis.
  • Collaborate with SummitStone staff and the Colorado Opioid Synergy Larimer and Weld (CO-SLAW) network.
  • Support clients navigating health systems and increase continuity of care.
  • Participate as part of a multidisciplinary team and assist where needed within the scope of the position.
