Care Coordinator – Acute Care

About The Position

Responsible for providing care coordination services to individuals transitioning from acute care settings. This role supports clients in meeting recovery goals through education, outreach, and connection to Medication Assisted Treatment (MAT) and additional community-based supports. Clients may be referred from emergency departments, primary care, crisis response teams, behavioral health providers, acute care facilities, and community health partners.

Requirements

  • High school diploma or completion of G.E.D required
  • At least one year of experience in care coordination or case management within the mental health field, substance use disorder field, or program area required
  • Experience with mild to severe mental illness and substance use disorders
  • Experience navigating systems of care
  • A desire to work with historically marginalized communities
  • Valid Colorado driver’s license or the ability to obtain one within 30 days of hire required; must maintain a clean driving record. This role requires occasional travel to client homes and community partner sites
  • CAT, CAS, CAC III, LAC or willingness to pursue preferred

Nice To Haves

  • Bachelors (BA/BS) Degree in psychology, social work or related field from accredited College or University preferred
  • Demonstrated experience working in a community-based behavioral health setting, providing direct support or services to individuals with mental health and/or substance use challenges preferred
  • Bilingual/bi-cultural preferred

Responsibilities

  • Provide care coordination services on behalf of and in collaboration with clients.
  • Engage with community partners including emergency departments, primary care, acute care systems, crisis response teams, and behavioral health teams to support continuity of care.
  • Outreach clients on a referral and/or cold outreach basis.
  • Participate in Certified Addiction Specialist coursework.
  • Collaborate with SummitStone staff and the Colorado Opioid Synergy Larimer and Weld (CO-SLAW) network.
  • Assist clients in navigating healthcare and community systems to promote successful discharge and follow-up care.
  • Assess client needs and collaborate with acute care teams to determine appropriate post-discharge services and supports.
  • Coordinate with internal teams and community agencies to identify resources, address service gaps, and support continuity of care.
