Requirements

  • Master’s degree in social work required.
  • Active social work license required (LSW).
  • Minimum of three months of experience in a related field.
  • Fundamental communication and problem-solving skills.
  • Ability to work independently and respond appropriately in crises.
  • Relate well to individuals with disabilities and older adults, respecting their privacy and dignity.
  • Effective and constructive interaction with diverse cultures and backgrounds.
  • Proficiency in Microsoft Office applications (Outlook, Word, Teams).
  • Strong verbal and written communication skills.
  • Valid driver's license, reliable transportation, and proof of liability insurance.

  • Preferred experience with program-specific populations.

Responsibilities

  • Implement the Bridge Transitional Care Model through provision of assessments through the Illinois Department on Aging, including intake, screening, assessment, case planning, crisis intervention, and monitoring/follow up.
  • Conduct comprehensive needs assessments and determine eligibility for services.
  • Establish written care or treatment plans with clients and families for maximum independence.
  • Active intervention and advocacy to access necessary services from various programs.
  • Complete monitoring, follow-up, and proper termination of cases.
  • Maintain confidential case records and meet documentation standards.
  • Accurate completion of all required reporting and documentation.
  • Maintain cooperative relationships with service providers and community organizations and educate community partners on clients ideal to participate in the transitional care program.
  • Coordinate with facility and medical providers to develop plan of care that will improve care transitions and reduce risk of readmission.
  • Provide monitoring and follow up of individuals who have discharged from facility to assure individual needs are being met and care plan is being implemented.
  • Attend training, staff, and team meetings, and act as an Agency ambassador in the community.
  • Other duties as assigned within the guidelines of this position.
