The Nurse Care Manager (RN) coordinates, negotiates, communicates, and manages the care of an assigned patient population along a continuum to facilitate optimal patient outcomes within appropriate length of stay (LOS) and resource utilization parameters.

Requirements

  • Graduate of a school of nursing
  • Two (2) to Three (3) years recent clinical nursing experience in an acute care setting
  • Current unrestricted License in good standing to practice as a Registered Nurse in the State of Connecticut
  • Current Basic Life Support (BLS) certification
  • Ability to obtain and maintain department specific certifications
  • Ability to read and understand oral and written instructions, and effectively communicate information
  • Basic computer skills (including opening/reading email, basic internet use)
  • Working knowledge of third party payors as well as hospital's registration and billing processes
  • Knowledge of health care fiscal management goals and strategies including trends and issues in health care reimbursement and knowledge of care delivery system across the continuum of care
  • Excellent interpersonal skills necessary to communicate with departments, employees, physicians and managers
  • Ability to manage time and meet deadlines in a hectic work environment
  • Ability to react and perform under strict timelines and/or emergency situations
  • Requires the ability to exercise discretion and use good judgment to perform duties promptly and consistently with little direction
  • Requires high level of analytical ability in order to gather and interpret information from various sources

Nice To Haves

  • BSN or MSN preferred
  • Three (3) years recent clinical nursing experience in an acute care setting with prior Care Management experience preferred
  • Certified Nurse Care Manager certification preferred
