Harbor is seeking an individual to manage, direct, and lead their assigned Care Coordination team. This position provides administrative and clinical supervision, and care coordination services based on client/family and agency needs. It is also responsible for working with local stakeholders to develop needed services and support within the communities served. Position is full-time, 40 hours per week, and works remote and in the community. Candidate will preferably live in Wood County.
