About The Position

As a Population Health Specialist, Certified Medical Assistant you will review, track and outreach to patients by phone on care gaps and preventative health items (no face to face patient contact) place standing orders as needed, facilitate and coordinate patient appointments, run daily hospital admission reports assessing for patient care needs, reconcile medications with patients after a hospital/skilled nursing facility discharge, communicate with insurance representatives on program measures, develop and maintain spreadsheets for multiple payer incentive programs, assess and provide documentation to address chronic conditions work, participate and take minutes for payer meetings. Requirements

• Graduate of a program for Medical Assistants or equivalent education.
• One year of experience working in a medical facility.
• Knowledge of medical terminology, common chronic diseases, and preventive health.
• Skill in medical record documentation.
• Ability to utilize independent and sound judgment to accomplish assigned responsibilities.
• Ability to self direct and multitask.
• Ability to be flexible and adaptable to changing assignments and work pace.
• Ability to recognize problems and share ideas for solutions.
• Sound judgment and decision making skills.
• Strong written, verbal and telephone communication skills.
• Excellent organizational skills with the ability to pay close attention to detail.
• Ability to work in a high volume, fast paced environment.
• Ability to establish effective working relationships with staff members, providers, patients and outside agencies.
• Ability to use a computer. Proficient keyboarding skills. Ability to type at least 30 words per minute. Microsoft windows proficiency, including Word, Excel, and Outlook.

Responsibilities

• Review care gap, Hierarchical Condition Categories and preventive health need reports.
• Have a thorough understanding of each payer’s incentive program and associated specifications.
• Research medical record to verify information.
• Develop and maintain a spreadsheet for tracking gaps, outreach, and outcome.
• Scan documents, create PDF files.
• Close gaps by uploading information to payer portal.
• Check for pending appointments; add care gap information to reason line of upcoming appointments.
• If no appointment pending, call patient to discuss care need and facilitate appointment scheduling if needed, inform them of needed tests.
• Enter test orders if standing order applies.
• Track outcome of appointment or test orders. Reschedule as needed.
• Document patient contact and outreach attempts in patient’s medical record.
• Send letters if unable to reach patient, or to communicate and summarize needs.
• Monitor for issues with non compliance (appointment cancellations, no-shows, failure to follow physician recommendations for care, etc.) Refer to CCC nurses for review.
• Notify Team Leader of noncompliance concerns with patients failing to follow-up after hospital or ED discharge.
• Communicate with insurance representatives by email and through their portals.
• Take meeting minutes for payer meetings.
• Contact patients to schedule Preventive Care Visits as needed.
• Contact patients to reconcile medications after hospital or Skilled Nursing Facility discharges.
• Cross train and provide back-up for the Behavioral Health Assistants.
• Provide back-up and support for CCC Assistant work as needed.
• Participate in the training of new hires as well as auditing work to ensure understanding and that workflows are being properly followed.
