As a Population Health Specialist, Certified Medical Assistant you will review, track and outreach to patients by phone on care gaps and preventative health items (no face to face patient contact) place standing orders as needed, facilitate and coordinate patient appointments, run daily hospital admission reports assessing for patient care needs, reconcile medications with patients after a hospital/skilled nursing facility discharge, communicate with insurance representatives on program measures, develop and maintain spreadsheets for multiple payer incentive programs, assess and provide documentation to address chronic conditions work, participate and take minutes for payer meetings.
