About The Position

As a Care Coordination Master Social Worker, you will play a vital role in providing comprehensive social work services to hospitalized and emergency department patients. You will be responsible for: Crisis intervention and patient/family support High-risk screening and brief counseling Connecting patients with financial and community resources Facilitating family/patient meetings with the healthcare team Arranging post-acute placement for complex discharges Addressing social determinants of health (housing, food insecurity, etc.) In this role you will have the opportunity to collaborate with members of the multidisciplinary care team, to help patients navigate their hospital stay and set them up for success as they transition to the next site of care. Requirements

  • Master's degree from a school of social work accredited by the Council of Social Work Education
  • 1-Year Post-MSW experience or Social Work internship in a clinical or medical setting.
  • Initial social work licensure as required by state law
  • Excellent communication, interpersonal, and problem-solving skills
  • Ability to work effectively in a fast-paced environment
  • Washington: Licensed Social Worker Associate Independent Clinical, or
  • Licensed Advanced Social Worker or
  • Licensed Social Worker Associate Advanced (MSW:WA)

Nice To Haves

  • Minimum 3-Year Post-MSW healthcare experience
