About The Position

Care Coordinator Associate Come and join the Commonwise Team! The role is full-time, salaried with benefits (one benefit being the most fun team you'll ever work with- we mean that!). The CCA will report directly to the Care Coordination Manager and assist with (and ultimately do) all she does. In a nutshell, Care Coordination is a combination of “air traffic control” and “match making.” It is a critical function ensuring the right caregiver shows up for the client on time. Caregivers provide their availability to work (and their preferences for client types) and clients provide their required times of care and support (and their preferences for caregivers). Care Coordination makes it all come together to everyone’s satisfaction. Does that sound stressful? It is! But if you love people and can deal with all types…if you are detail oriented (like obsessively so) and love solving problems…if you are ablaze with drive, stamina and don’t need a lot of supervision…. then, read on. Some of our clients are suffering from dementia and other debilitating illnesses. They are scared and sometimes take it out on others, as do their adult children. While that is not “ok,” nevertheless, care coordination needs to overlook the occasional jab and be ever focused on solving what problems can be solved. Requirements

  • Knows how to recharge and take care of self.
  • Impeccable attention to detail and a commitment to quality work product.
  • Extremely organized; able to manage a myriad of competing priorities
  • High energy, responds well to pressure, works to deadlines
  • Ability to work on own initiative as well as with a team
  • Proven communication and interpersonal skills necessary for community building internally, and trust building externally
  • Must have integrity, tenacity, and credibility complimented by a bias for action and a strong sense of personal accountability
  • A joyful approach to work and life combined with a sense of humor highly recommended
  • Loves chocolate or its equivalent
  • Education: GED required, Associate or BA preferred
  • Experience: General life experience (highs and lows) a must.

Responsibilities

  • Mastering the software “Wellsky” – the central repository of all of the clients and caregiver information.
  • Working with clients and caregivers (over the phone, email and text) to ensure shifts are filled and feedback collected.
  • Solving emergent problems.
  • Tact and social grace (patience) is required when dealing with caregivers who are in need of “professional coaching.”
  • Collaborating closely with other team members such as the clinical and recruiting team.
  • Surrendering a weekend or evening now and then – especially when there is inclement weather or your care coordinating colleagues take a vacation.
  • Doing a lot of administrative activities.
  • Answering phones.
  • Calling clients and caregivers.
  • Tracking down paperwork and uploading it to various files.
  • Running out to grab lunch for your colleagues when they forget their lunch.
  • Texting colleagues asking them to bring you lunch as you are slammed and can’t leave your desk.
