About The Position

Requirements

  • High School Diploma or GED, (Required)
  • Valid Drivers License Required

Nice To Haves

  • Associates Degree, (Preferred)
  • 1 year Healthcare related experience (Preferred)
  • Certified Nurse Aid (CNA) - Virginia Department of Health Professions (VDHP) (Preferred)
  • Licensed Practical Nurse (LPN) - Virginia Department of Health Professions (VDHP) (Preferred)

Responsibilities

  • Facilitate patient access to various community resources while collaborating with social services agencies and medical providers.
  • Educate patients on health management, the appropriate use of healthcare services (e.g., emergency room alternatives), and encourage active participation in their health and well-being.
  • Work closely with an interdisciplinary team, including Physicians, Nurses, Care Managers, Social Workers, and Health Educators and others, to improve patient health outcomes and address care gaps.
  • Conduct telephonic outreach to engage members in care, address service coordination needs, close care gaps, and connect members to educational and community support programs.
  • Maintain accurate and comprehensive records of client interactions, service plans, and outcomes in electronic health records, submitting monthly reports as required.
  • Build positive relationships with clients, providers, and community organizations, working to reduce cultural and socio-economic barriers that impact access to care.
  • Participate in community events and activities to foster relationships, understand community health issues, and promote awareness of available resources.
