About The Position

This position coordinates comprehensive rehabilitation services for patients recovering from cardiac events, CABG procedures, and other high-risk conditions. The Cardiac Rehabilitation Specialist implements evidence-based secondary prevention strategies, including individualized exercise programming, patient education, and risk reduction interventions. This role also provides rehabilitative services for postcardiac and pulmonary patients, as well as individuals with multiple cardiovascular risk factors. At Logan Health, our Cardiac Rehabilitation team plays a vital role in guiding patients through recovery after heart surgery, cardiac events, pulmonary conditions, and high-risk diagnoses. As a PRN Cardiac Rehabilitation Specialist, you’ll help patients regain function, reduce risk, and improve quality of life — one personalized plan at a time.

Requirements

  • Bachelor’s degree in a health-related field OR Associate’s degree in Respiratory Therapy
  • CEP or CCRP certification within one (1) year of hire
  • Current Healthcare Provider CPR & AED certification
  • ACLS preferred
  • Prior healthcare experience preferred

Responsibilities

  • Coordinate rehabilitation services for post-cardiac event, CABG, pulmonary, and high-risk patients
  • Implement secondary prevention strategies including exercise programming, education, and risk reduction
  • Conduct pre-exercise assessments and develop individualized treatment plans
  • Utilize American Association of Cardiovascular and Pulmonary Rehabilitation (AACVPR) standards for screening and program delivery
  • Provide patient education on heart health, lifestyle changes, and chronic disease management
  • Coordinate referrals, verify insurance coverage, and collaborate with referring providers
  • Support accurate documentation, billing, and census reporting
  • Cross-refer patients to programs such as smoking cessation, diabetes education, and heart failure clinic
  • Work closely with providers, patients, and interdisciplinary team members to ensure safe, effective, and compassionate rehabilitative care.
