About The Position

The Cardiac nurse navigator provides a personalized level of support to help patients, families, and caregivers navigate the system of care, at all levels of care, and assists them in maximizing their care. The Cardiac nurse navigator performs screenings for atrial fibrillation or structural heart disease, reviews and escalates incidental findings, and acts to strengthen the cardiology program. In collaboration with the multidisciplinary team, she/he is responsible for coordinating timely access to quality and comprehensive services throughout the continuum of care. This position serves as a clinical resource with expertise in care management. The Cardiac nurse navigator also serves as a liaison throughout AtlantiCare and in the community. The Cardiac nurse navigator also provides clinical support and acts as a patient advocate with the caregivers that are both internal and external, to support wellness and optimal health in the community. She/he will be responsible for harnessing artificial intelligence to aid in surveillance and referral of patients in to the cardiac service line. This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to teamwork, program growth, and professional cooperation.

Requirements

  • Graduate of an accredited school of nursing
  • Current licensure as a Registered Nurse in the State of New Jersey
  • BLSHCP Certification required 6 months from date of hire or transfer
  • Excellent interpersonal skills
  • Motivational interviewing skills
  • Excellent written and verbal communication skills
  • Computer literacy
  • Proficiency in Microsoft Office Applications -- Excel, Word, and PowerPoint
  • Ability to communicate professionally and effectively with individuals or in a group setting on the clinical effectiveness of new products and technology
  • Organized, detailed oriented
  • Good time management skills to succeed in a fast paced and ever changing work environment

Nice To Haves

  • Bachelor's in nursing preferred, or in active pursuit
  • 5 years cardiology, critical care, Cardiac Cath Lab or Electrophysiology clinical experience preferred
  • Proficiency in Clinical Applications preferred at time of hire; incumbents within position will be trained appropriately on the skills that will be required for this position within 30-60 days from date of hire
