About The Position

Provide basic case management services to medically fragile children who are at risk of institutionalization and whose health, safety, and well-being can be maintained in his/her primary residence. Coordinate services with RN to ensure quality care. Responsibilities include: Establish eligibility for services, obtain client histories, perform initial assessments, develop a plan to provide basic services, explain available services and programs, and refer clients to appropriate programs/agencies for other needed services. Evaluate and document client programs Conduct interviews and work closely with clients, families, other relatives, school, medical providers, and other community agencies contacts to obtain information to be incorporated into a social history which will aid in planning for required services as appropriate to the individual plan of care. Provide basic case management services as the lead member of a Social Worker/RN interdisciplinary team.
Requirements
- Graduation from an accredited School of Nursing.
- Ability to direct and carry out prescribed medical treatments and therapeutic programs.
- Thorough knowledge of professional nursing theory and practice.
- RN certification with one year of nursing experience preferred.
- Valid NC driver’s license and a clean driving record (this is a condition of employment)
- Personal auto liability insurance is mandatory if transporting clients in private/personal vehicle(s).
- Must meet all other conditions of employment related to criminal record screening, drug screening, reference checking, etc.
