This position provide basic case management services to medically fragile children who are at risk of institutionalization and whose health, safety, and well-being can be maintained in his/her primary residence. Requirements

• Required Education Graduation from an accredited School of Nursing.
• Ability to direct and carry out prescribed medical treatments and therapeutic programs.
• Thorough knowledge of professional nursing theory and practice.
• RN certification with one year of nursing experience preferred.
• Required Licensure/Certifications Valid NC driver’s license and a clean driving record (this is a condition of employment)
• Personal auto liability insurance is mandatory if transporting clients in private/personal vehicle(s).
• Must meet all other conditions of employment related to criminal record screening, drug screening, reference checking, ect
• Essential Physical and Sensory Requirements-With or without the aid of mechanical devices Regularly required to lift 30 lbs.
• Must be able to lift a minimum of 70 lbs.
• Must be able to pull minimum of 50lbs.
• Must be able to squat, kneel, crawl, crouch, climb, and stoop.
• Must be able to regularly use hands to finger, handle, or feel objects, tools, or controls.
• Required to regularly stand and walk.
• Must be able to demonstrate proficiency in CPR from the floor level requiring to work on hands, knees, bending, standing and lifting.
• Vision requirements include close vision, distance vision, and peripheral vision.
• Must be able to talk and hear

Responsibilities

• Establish eligibility for services, obtain client histories, perform initial assessments, develop a plan to provide basic services, explain available services and programs, and refer clients to appropriate programs/agencies for other needed services.
• Evaluate and document client programs
• Conduct interviews and work closely with clients, families, other relatives, school, medical providers, and other community agencies contacts to obtain information to be incorporated into a social history which will aid in planning for required services as appropriate to the individual plan of care.
• Provide basic case management services as the lead member of a Social Worker/RN interdisciplinary team.
