Shift Monday - Friday, Days Scheduled Weekly Hours 32 POSITION PURPOSE: Enhance the quality of patient care by addressing the psychosocial needs of patients and their families by providing assessment, intervention, education and referral to appropriate resources. Interventions may be with all age patients and their families/caregivers including but not limited to psychosocial assessment, brief individual or family, advocacy, linkage, referral, group work, case coordination, and long-term care planning and placement (survivorship).

Requirements

  • Bachelor's Degree in Social Work or related field required; Masters' Degree in Social Work preferred.
  • Current Illinois Licensure as Social Worker (LSW) or Clinical Social Worker (LCSW) preferred.
  • 3 years of acute care, long-term care, discharge planning/case management preferred.
  • Current knowledge of medical care/treatment, JCAHO standards, Federal/State regulations, i.e. Medicare, Medicaid, and third party payer requirements relative to discharge planning preferred.
  • Valid Illinois Vehicle Operator's License required

Responsibilities

  • Protect and promote patients' rights through education and advocacy.
  • Provide education to patient and family about the variety of resources and options which will enable the individual to return to the highest level of functioning.
  • Documents assessment findings and social service interventions appropriately in the medical record and communicates same to the appropriate health care team members.
  • Develops, implements, evaluates and revises, as necessary, a plan for discharge, including referrals to other health care and community organizations based on needs assessment.
  • Works collaboratively and professionally with patients, family members, physicians, hospital staff and other individuals and agencies involved in providing patient care.
  • Assists patients/family members in obtaining Advanced Directives and/or Durable Power of Attorney for Health Care as needed.
  • Provides patient and/or family counseling to assist with coping, grieving, conflict resolution, and crisis intervention
  • Acts as a resource and liaison between patients, family members, physicians, interdisciplinary team and external community agencies and organizations.
  • Utilizes knowledge and understanding of CMS regulations, Social Security benefits, managed care, and other payer regulations and benefit limits when providing education or intervention with patients and families.
  • Completes job duties and serves internal and external customers efficiently and resourcefully to result in customer satisfaction and cost effectiveness.
  • Able to consult and coordinate with multidisciplinary team of care professionals and oncology site medical/nursing/MA staff, to include family conferences, care conferences, team meetings.
  • Collect data relevant to visit with ongoing assessment throughout the duration of care. Evaluates patients/family needs through timely assessment, communication and observation. Modifies plan of care through assessment based upon patient/care systems response.
  • Evaluates psychosocial assets and deficits for the purpose of enabling the patient to maintain the greatest level of independence and wellness.
  • Provides a wide range of services in the oncology setting, including support and consultation to patient and care team throughout the disease continuum.
  • Coordinates with others to establish goals of care.
  • Performs psychosocial triage, teaching and coordinating with other resources for care giving needs. Making referrals when appropriate.
  • Be able to provide real time counseling one day per week in department and refer to Ways to Wellness.
