About The Position

We're seeking a Case Manger to join our Mobile Crisis Team. The Mobile Crisis Team is a two person team available 24 hours a day, 7 days a week, 365 days a year to respond to Cullman County residents in a mental health crisis. Their goal is to assess crisis needs and then work to resolve the immediate crisis either through direct crisis intervention or warm hand-off to the services needed to resolve the crisis. A key to a successful outcome is continuous monitoring of the status of clients being served from initial call to warm hand-off. Those hired in this position will be working 3rd shift Monday-Friday, 11:00PM - 7:00AM. What you’ll be doing: Conduct individual and/or family therapy with children and adolescent population ages three to eighteen. Work with Program Manager and Therapist to assist families on a 24/7 call basis. Provide Crisis Intervention for client and client’s family when called. Perform initial assessments, intakes and prepare documentation of such for client’s Electronic Medical Record (EMR). Prepare with client a treatment plan to include goals. Facilitate warm hand-off to Therapist and/or Care Coordinator. Obtain certification in the CANS System. Conduct CANS Assessments. Work closely with Therapist, Care Coordinator & guardians regarding client’s needs. Participate in various meetings to include departmental, divisional, or other meetings as required. Complete billing documentation in a timely and accurate manner. Maintain educational requirements regarding licensure, Alabama Department of Mental Health State Standards Perform outreach in partnership with DHR. Complete other duties as assigned. Requirements

  • Bachelor of Arts/Science, preferably in a human service field
  • Enjoy working with youth/young adults
  • Valid Alabama driver’s license and a suitable automobile to permit the required travel.
  • Must show car insurance equal to or more than the State of Alabama standards for automobile insurance coverage
  • Must become certified as a Care Coordinator (adults and children) by completing DMH-approved trainings
  • Must understand and endorse a recovery-oriented approach to providing services
