About The Position

Stella Maris Hospice, located in Timonium Maryland, is looking for a Bridge Program Manager (RN) to oversee the development, coordination, and administration of the Palliative Care Bridge Program at Stella Maris. The goal of this position is to focus on outreach and education to connect patients and caregivers to critical services that are a good fit for them. This is an exciting opportunity to lead and develop this program to meet the needs of clients in the community. Bridge Program Manager (Registered Nurse) Palliative Care Bridge Program at Stella Maris Full-time, Monday-Friday 8a-430p Responsibilities Under the general direction of the (TBD) Administrator, this position is responsible for overseeing the development, coordination, and administration of all aspects of the Palliative Care program at Stella Maris. These components focus on connecting the elderly/ill/disabled and their caregivers to critical services. Duties will include planning, organizing, staffing, and leading program. Additional responsibilities may also include working with the liaisons on the overall outreach and education efforts of the program.

Requirements

  • RN license in Maryland (BSN preferred)
  • Hospice experience is required and/or five (5) years of `recent experience in a medical/surgical setting, a hospital, oncology or geriatric facility.
  • Ability to effectively present information and respond to questions from groups of government officials, stakeholders, executives, clients, families and the general public.

Responsibilities

  • Plan & Organize Program
  • Identify, cultivate, and sustain meaningful partnerships with area healthcare providers to spread awareness about the Bridge Program at Stella Maris.
  • Identify services needed through a community needs assessment.
  • Collaborate closely with relevant community entities including but not limited to Stella Maris Home Care and Hospice team members as well as partnering providers and hospitals.
  • Determine best practices in executing program and identify barriers and service issues that will effect implementation of program.
  • Oversee the collection and maintenance of client records for statistical purposes according to the confidentiality/privacy policy of the organization.
  • Assist in conducting monthly, quarterly, and annual assessments of programs; identify weaknesses, create mitigation plans and assist in implement to improve quality of programs to maximize outcomes.
  • Implement Program
  • Performs initial patient assessment including documenting assessment, communicating with interdisciplinary team members, obtaining initial orders, completing initial home/resident evaluation, conduct patient/caregiver education, completing initial plan of care, acting as a resource to members of the interdisciplinary team.
  • Supports the RN Case Managers in providing case work services
  • Supports the Hospice QA program by performing activities as assigned.
  • Manages patients who will be admitted to Palliative Care.
  • Communicates effectively changing needs of palliative patients to Administrator and hospice staff.
  • Participates in interdisciplinary team meetings to assist with evaluation of Palliative patients continue care.
  • Evaluates plans of care of palliative care patients.
  • Assists with other nursing and clinical staff duties as directed by the Administrator and supports on-call process and schedule.
  • Departmental Leadership Responsibilities
  • Coordinate the delivery of services among different program activities to increase effectiveness and efficiency
  • Actively seek opportunities to engage with affected population
  • Serve as community liaison and representative with community organizations, stakeholders, providers, and town, city, and county governments regarding programming around home health, palliative care, and caregiver support.
