The Oregon Department of Human Services is seeking a dedicated and compassionate Brain Injury Services Case Manager to join our team. As a Case Manager, you will engage with service applicants, their families, and other stakeholders to assess needs and determine eligibility for Brain Injury Services and Programming. In this role, you will be responsible for ongoing case management, which includes coordinating services according to established rules and regulations, conducting monthly service contacts, and making community referrals as needed. You will document all aspects of each case meticulously using various systems, including the mainframe system (DHR), the ONE eligibility system, and the Medicaid Management Information System (MMIS). Your attention to detail and commitment to accurate data entry will be essential in ensuring that clients receive the benefits and support they are entitled to. The position offers the flexibility of remote work, although occasional reporting to the Salem office may be required based on operational needs.
