About The Position
We are seeking a Blended Case Manager to join our team in our Sharon Hill, PA location. The Blended Case Manager (BCM) will provide resource coordination, recovery services, and symptom management using a strengths-based model of case management to 20-25 adults living with a mental illness. This role is crucial in ensuring that individuals receive the necessary support and resources to manage their conditions effectively. The BCM will also provide information and support to families and significant others of the individuals served, helping them understand the challenges faced by their loved ones and how best to support them. Additionally, the BCM will be responsible for completing required paperwork, correspondence, and chart documentation to ensure compliance with organizational standards and regulations. The position requires a commitment to working collaboratively with clients, their families, and other service providers to create a comprehensive support plan tailored to each individual's needs. The BCM will engage in regular assessments and evaluations to monitor progress and adjust services as necessary. This role is not only about providing direct support but also about empowering clients to take charge of their recovery journey and improve their quality of life. The BCM will work full-time, Monday through Friday, from 9:00 AM to 5:00 PM, and will not be required to work on weekends, allowing for a balanced work-life schedule.
