The Case Manager will work with medical providers and nursing staff to identify potential clients, assess, and deliver supportive services to clients. Requirements

  • Associate’s degree in social work, child development, community health, or related field required.
  • Intermediate proficiency in Microsoft Office required.
  • Local travel required, valid Illinois driver’s license and automobile insurance also required.
  • Cultural competency to work in a low income, minority environment.
  • Ability to maintain confidentiality and trust from clients.
  • Ability to establish and maintain effective working relationships with customers, clients, and community partners.
  • Ability to work well individually and as part of a team.
  • Record keeping methods, practices, and procedures.

Nice To Haves

  • Bachelor’s degree in social work, child development, community health, or related field preferred.
  • A minimum of two years' work experience in health, social or community services preferred.
  • Bilingual fluency (English/Spanish) preferred (may be required to assist or work with non-English/Spanish-only speaking patients and/or guests.)

Responsibilities

  • Conducts outreach, advocacy, assessments, care plan, home visitation, and EMR data entry.
  • Collaborates with fellow Case Managers to provide constant floor coverage throughout the day.
  • Displays familiarity and knowledge of the care plan of each client and facilitates referrals and follow-up process.
  • Follows the Department of Human Services Maternal and Child code requirements.
  • Acts as a mandated reporter to the Department of Children and Family Services.
