About The Position

The Home Visitor conducts one on one parenting education and community resource referrals in the homes or community settings of families with children ages 0-17, develops service plans, facilitates assessments and screenings, utilizes and follows program model to fidelity, maintains client chart & computer chart records, attends regular trainings and meetings, conducts outreach to engage families in the program and facilitates Family Resource Center events, classes, groups and activities.

Requirements

  • High School Diploma or equivalency required.
  • Two plus years related client experience, including home visitation experience, OR an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job.
  • Strong administrative, time management and organizational skills.
  • Clear, effective and respectful verbal and written communication skills.
  • Knowledge of child abuse issues and family dynamics, substance abuse issues and treatment, domestic violence issues and resources, community resources, and human development.
  • Ability to work with a team and independently.
  • Ability to present information in a group setting in a clear and concise manner.
  • Ability to work set schedule with occasional evenings and weekends.
  • Demonstrates computer skills including ability to utilize word processing, spread sheets, email and calendar, and various documentation websites.
  • Willing to use own transportation to and from the office, home and community visits and assigned trainings.
  • Ability to establish rapport and maintain personal and professional boundaries while providing supportive services.
  • Strong interpersonal skills and the ability to relate to individuals who may not share basic commonality, including value systems and behavior norms.
  • Ability to provide culturally and linguistically appropriate services.
  • Maintain a professional and confidential work environment.
  • Self-initiate multiple tasks in an efficient manner.
  • Strong stress management skills.
  • Adherence to infection control procedures, including but not limited to, standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking and social distancing.
  • Assist and support the Center/Department/Program to meet standards of High Reliability.

Nice To Haves

  • Bachelor’s degree in the field of Social Work, Child Development or Psychology preferred.
  • Bilingual fluency in written, and spoken language in English and one of the following is preferred: Farsi, Dari, Pashto, Russian or Spanish.

Responsibilities

  • Knowledge, understanding and willingness to learn of the values, beliefs and practices of cultural groups within diverse communities.
  • Advocate for and provide guidance to families with resources, systems and cultural brokerage.
  • Conduct regularly scheduled home and community visits to assigned families and children.
  • Document notes from visits with assigned families and children.
  • Provide and adhere to the program curriculum and model, utilize program assessments and screenings, facilitate parent-child activities and bonding, provide health information at every visit as identified by the program model.
  • Plan and facilitate groups and classes in the Family Resource Center.
  • Reflect those who reside in the identified service neighborhoods.
  • Work with families to empower them to engage in problem solving.
  • Model effective parenting behavior reflective of program standards and curriculum, provide support, education and information, age appropriate parenting and positive discipline techniques.
  • Promote parent participation in activities designed to promote the physical, emotional, social and intellectual development of their children.
  • Partner with families to develop, renew, and update service plan per program policies and procedures under the supervision of the assigned supervisor.
  • Model nurturing attitudes and behaviors in all contacts and representations of the organization.
  • If bilingual, translate curriculum and provide services in the language of the family served.
  • Complete all documentation as required by policies and procedures of the organization and program.
  • Present family strengths and needs at multi-disciplinary resource team meetings, collaborate on and implement family service plans with team, supervisor and families.
  • Actively participate in weekly individual supervision meetings with Team Leader or Family Resource Center Coordinator.
  • Actively participate in staff meetings, Family Resource Center events, and required trainings at the organization and off site.
  • Adhere to Mandated Child Abuse Reporter laws.
  • Maintain clean and organized office and Family Resource Center.
  • Facilitate play-care as scheduled.
  • Assist in outreach and engagement of new family participants and community resources.
  • Ensure clean data recording and time studies.
  • Travel between program office, family homes and community visits daily using own vehicle.
  • Perform other duties as assigned.
